Part two of the ladder · Vernon Alcott

You found your rung. What I want back for you is a door left open at your daughter's.

The ladder was free and it was only diagnostic. It told you which rung you are standing on. It said nothing about why a rung never comes down on its own, why the oregano bottle in your cupboard was never actually a test, or what you are choosing between this week. Not fewer symptoms. That spare room, or four more years getting better at force.

By Vernon Alcott, retired radiologic technologist, Little Rock, ARSat. Jul. 25th, 20267 minute read
A bathroom sink in the half dark of early morning, tap running
Between 5:40 and 6:00, five to ten minutes, tap running. In twenty four years I never saw a requisition ask how many minutes or what colour. The tap is not for the water. It is so the house cannot hear you.

Most of you found your rung in about four seconds. Bathroom, 5:40 to 6:00, five to ten minutes because the stuff in there is sticky and hard to dislodge, that is rung three. Afternoon flight automatic, no staying at your daughter's because of the walls, back seat at church, that is rung four wearing rung three's clothes.

I ended the ladder by telling you to decide whether you wanted to spend the next four years getting better at force. That was not a flourish. It is the actual choice in front of you, and the reason is on this page.

Relief and an answer are two different objects, and you were handed one of them.

Now put the rung down, because a stage on a page is not what you want back. What you want is a Thursday in November where you sleep in your daughter's spare room with the door open, and you are not lying there at 5:20 doing arithmetic about how thin that wall is. What you want is an 8:15 flight booked without a second thought. Not fewer symptoms. That room, that door, open.

Two more things I should say before the mechanism, and the first one is about the topic itself. Women write to me about this and open with some version of I know this is not the most attractive subject, so I apologise in advance. You will not do that here. You are describing a cleanup job with a start time and a duration, and there is no version of that which is your fault or beneath discussion.

The second is that you have every reason to be sour about a page on a phone, because nobody knows anything and everybody is confident. The primary says sinuses. The ENT says it is a GI issue. The gastroenterologist says he has never heard of it. Many tests and x-rays, a clear film, the word manage, and a bathroom you cannot leave until six.

It is difficult to be with people, because they say why are you clearing your throat all the time.

That sentence, or one within a word of it, is the thing I have read most often from women on rung three, and it is the same joke your family made at rung one before either of you understood what you were looking at. You told them to stop. They were the first instrument that ever detected this, years ahead of any machine, and nobody wrote down what they found.

So I am not going to ask you to trust me. I am not a radiologist and I am not a physician, and I said so in the first ten lines of the ladder. Twenty four years behind the glass and something over forty thousand chest images does not make me your doctor. It makes me the man who stood there while the same woman in the same gown asked the same question. So the first real thing on this page is a test you run on your own cupboard tonight, on a bottle you already paid for, and it needs nothing from me at all.

What this page covers

  • Two things you can settle tonight without me. The fifteen seconds on the bottle in your cupboard, and the ninety seconds on the report in your drawer.
  • Why everything on your shelf moves force or width, and none of it touches why the layer keeps arriving.
  • An honest calendar. Nothing in weeks one to three, first change in weeks four to six, as a clock and not a feeling.

Rung four is not a symptom. It is an invoice.

Rung three is minutes. Rung four is what you paid to keep rung three private, so long that none of it feels like a decision any more.

The afternoon flight, always. You have not looked at an 8:15 in years and you would tell anybody who asked that you simply prefer flying later.

Not staying at your daughter's, because of the walls. She thinks it is your back. You have let her think that.

The back seat at church, after years of sitting four rows from the front, moved without announcing it so nobody would have to sit next to it.

And the water in the car for the fifteen minutes after you park, which means you now arrive everywhere fifteen minutes before you arrive.

Empty pews photographed from the back of a church
Nobody moves to the back of a church by accident, the same way nobody buys a wedge pillow by accident. Those four arrangements are the only staged record of the last six years that exists, because the machine does not make one. You built it and reported none of it.

Why a rung does not come down on its own

Your airway is meant to carry a thin film of mucus, with cilia underneath sweeping it upward all night so you swallow it without knowing. That escalator has run quietly your whole life.

Years of irritation change the equipment. Mucus cells multiply, the glands enlarge, the film thickens into a coating. The cilia were built to move a film, not a coating, so they are overwhelmed, in patches damaged, and they stop beating. The escalator stalls, and one mechanism is left for moving what is in there.

Force. Force is the sink. Force is the tap. Force is the ten minutes, and it is the reason it is ten and not two.

Here is how you can tell that is the right description rather than a nice metaphor. Once you get it out in the morning, that is it for the day. It does not come back until the next morning. That pattern is not what an infection does and it is not what an allergy does. It is a night of accumulation being removed by hand in one go, and then eighteen hours of quiet accumulating the next one. You have been running that experiment on yourself every day for six years and nobody ever asked you the result.

Which is also why the two questions I never once saw written on a requisition are the only two that stage it. How many minutes, and what colour. You can answer both right now. The system has never asked, and it is not going to, because there is no field on the form for it.

Illustration of an irritated airway lining with a thickened coating on the inner wall
Two levers, and almost nobody separates them. The tube is the width of the pipe, which is what an inhaler opens. The layer is the coating inside it. Opening a pipe does not clean the inside of a pipe.

Now go and read your own shelf

Not to feel foolish. Every item on it does the job printed on its box. Look at the job, not the hope.

The humidifier, then the second humidifier, then the steam and the hot shower with the door shut where the water covers the sound.

All one lever. Wetter air, so the coating shifts more easily when you shift it. This is what I watched women go home and buy after a clear film, for twenty four years.

Makes force more effective, one morning at a time. Changes nothing about why the layer arrives.

The wedge pillow.

Gravity. It stops the coating pooling overnight, which is why lying flat starts the rattle. One pillow became two, two became a wedge, and it felt like bedding, so you never reported it.

Repositions the material. Does not reduce it.

The expectorant.

Read the back of it. It helps make coughs more productive. Honest labelling, doing exactly what it says, which is thinning the material so force can move it.

Thins the layer for the ten minutes. Says nothing about tomorrow morning.

The inhaler, if you carry one, and the huff the physiotherapist taught you.

The inhaler opens the tube, fast and real, one of the genuinely good tools in medicine. Keep every prescription exactly as prescribed. The huff is a good technique taught well, and it is still you, applying force.

One widens the pipe. One improves the force. Neither cleans the inside of the pipe.

Four rows, two levers, not one aimed at the irritation that keeps ordering the layer. Which is why six years of doing everything right moved that clock by zero minutes. You did not fail at this. You were never handed the lever worth failing at.

Fifteen seconds, in your own cupboard, before you believe a word

There is one more and it is not on the shelf. It is in the cupboard behind the cinnamon. Somebody suggested oil of oregano once, you took it six weeks, nothing happened, and you filed the category under tried it.

Go and get it. Turn it over. The amount first, and most oregano capsules sit between 150mg and 500mg, a seasoning quantity, what you would put on a roast for four. Then the one that decides this: look for the word carvacrol and a number beside it. It is the active compound and the whole reason anybody takes oregano oil. On most of those bottles there is no carvacrol figure anywhere.

A hand holding a supplement box, reading the small print on the back panel
The back panel settles it, on the expectorant box and the oregano bottle both. Two batches of oregano carry wildly different amounts of carvacrol, which is why a company that measured it prints it.

Now think about what I told you on the ladder. The layer is not absent, it is not visible, and those two sentences are completely different. A blank on that panel is the same kind of sentence. It does not say there is very little carvacrol in there. It says nobody measured it, and I was trained to treat unmeasured as unknown, never as fine.

So those six weeks were not a failed experiment. They were not an experiment. You took an unstated quantity of an unstated compound and wrote a conclusion about the compound. The same mistake the system made with your chest, in your own hand, with your own money.

The lever nothing on that shelf pulls

If the layer is a response to irritation, the only lever with a chance of changing the picture acts on the irritation itself. Not the width of the tube. Not the wetness of what is in it. The reason it keeps being made while you sleep.

The two compounds with a genuine research literature on that side are carvacrol and thymoquinone, the primary actives in oregano oil and black seed oil, nigella sativa. Neither is a drug, neither is approved to treat any lung condition, and neither cures anything. If you land on a page telling you a supplement cures a lung disease, close it. That is not a grey area.

What it does is act on the one side of this nothing in your house is pointed at, which puts you back at a panel. Three things have to be printed on one before a bottle is a trial rather than a gesture.

What has to be on the label before it counts

  • Amount. A seasoning dose is not a trial dose. The one I take states 6,000mg of wild oregano and black seed oil per serving.
  • Stated actives. The carvacrol and thymoquinone content is printed on the label, in the exact field where your bottle is blank.
  • Oil delivery. Carvacrol is fat soluble and has to travel in a lipid. Leaf powder in a hard capsule brings no carrier. An oil in a softgel brings its own.

It is called Veylor. I am naming one instead of saying go and buy oil of oregano and good luck, which is how you ended up with a bottle behind the cinnamon that proved nothing.

Veylor oregano and black seed oil softgel pouch with the supplement panel visible
Two softgels at night, with water. At night because the coating rebuilds while you sleep, which is why the worst of it is waiting at 5:40 before you have said good morning to anybody.

Ninety days, honestly, because the rung took six years to climb

I will not tell you about week one. Anybody promising week one is selling you the same ninety second relief the phone call sold you.

Weeks 1 to 3

Nothing at the sink. Same minutes, same colour, same clock. This is where most people quit and then spend four years telling other women they tried it.

Weeks 4 to 6

Where the first change shows up if it is going to, and it is the clock rather than the feeling. Ten minutes becomes seven, and you notice on the morning you are dressed earlier than you meant to be.

Weeks 7 to 12

The pattern matters more than any single morning. Fewer mornings that own the whole first hour, and the honest read is at ninety days. Whatever you have by then is your answer. The irritation was never a one time event, and the people who keep their mornings keep taking it.

Keep every prescription exactly as prescribed the whole way through. That calendar is why the guarantee runs ninety days and not thirty.

And do one thing before your first softgel or you will never know. Tonight, on paper by the bathroom light switch, write the minutes and the colour. Change nothing else, and read it in week five.

Why this week, and not in October

A wall calendar with the early mornings marked off
There is no countdown on this page, there is a calendar. Every morning you wait is another ten minutes of you doing by hand, at 5:40, a job an escalator used to do for free.

Ten minutes, seven mornings, is seventy minutes a week with the tap running, so eight weeks of deciding is nine hours. Four more years of it, which is the sentence I ended the ladder on, is somewhere near two hundred and forty hours. Ten days. Ten whole days of your life, standing up, in the dark, before anybody has said good morning to you.

And the ninety days cannot begin until a box is in your hand. Start this week and the honest read lands in late October, which puts Thanksgiving on the far side of it. Start in October and it arrives after Christmas, with the whole winter inside the trial.

So pick the date you are actually buying. Thanksgiving at your daughter's, the spare room, the door open, you asleep in it at 5:20 instead of awake and calculating how thin that wall is. An 8:15 flight in the spring, booked the way you used to book one, without the arithmetic. Your old seat, four rows from the front, instead of the back row you moved to so nobody would have to sit next to it.

On the price, and there is no clock on this page and I will not invent one. Wild oregano is harvested, not manufactured, and the carvacrol in a batch depends on the hillside it came off. The number on the label belongs to that harvest, and the sixty percent is priced against the batch in the warehouse now.

The questions, in the order you are asking them

I already tried oregano. Six weeks, and absolutely nothing happened. So why is this any different from the bottle sitting in my cupboard right now.

Get that bottle first. If it prints a carvacrol figure and the dose is comparable, you ran a real test, you have a real answer, and you should not buy this. If the field is blank, you did not run a test. You cannot conclude anything about a compound from a bottle that never said how much was in there.

Nobody has ever been able to tell me what this actually is. Many tests, many x-rays, and no one knows anything. So how does a page on my phone know.

It does not, and I am not handing you a diagnosis. What is on this page is narrower than that, and all of it is checkable by you. Your own shelf shows four rows of force and width and nothing aimed at the irritation. Your own bottle shows a blank where the carvacrol number should be. Both of those you settle tonight without me. If a doctor has told you what this is, that answer stands and nothing here changes it.

Will this interfere with my inhaler, or the rest of what I take every morning.

A food supplement, not a drug, sitting alongside everything you already take. Keep every prescription exactly as prescribed. If you are on a blood thinner, or have surgery coming, let whoever manages that say yes first.

I have been fooled before, and I run four things at once, so I will never know which one did anything.

Best objection on the page, and why I gave you the paper by the light switch. Change one thing. Seven interventions at once is how women end up better and still unable to say what worked. On the money, the guarantee runs ninety days from delivery, which is the same length as the test, so the only thing you are actually putting up is the writing down.

Say it does something. What actually changes.

Minutes first, and only minutes. Ten becomes seven, then the first hour of the day stops belonging to a basin. It cures nothing and is not approved to treat any lung condition. What comes back after the minutes is not medical at all. The 8:15 flight, your old seat, a door you can leave open.

P.S. Two things tonight that cost nothing. Get the report out of the drawer and look for any sentence about a layer, or minutes, or mornings. There is none, on any report, because there is no view on that machine that makes one. Then get the bottle and look for the carvacrol number. The first says the system never staged this. The second says you never tested it.

P.P.S. I am on rung two myself and only found that out by writing the ladder. Wedge pillow since 2022, never once filed as a medical event. That is how quietly this moves.
